Contractor Badges
All external contractors, vendors, and clients must show identification when on University of Oregon property. These badges are required display the individual’s photo, the name of their organization, their job title, and phone number. If the contractor/vendor organization has already supplied a badge which meets these requirements, no additional UO badge is needed. If a vendor or contracted employee does not have a badge which meets these requirements, they may purchase one from the ID Card Office.
To request vendor or contractor badges, you will be asked to provide the following:
- Name of the individual working on site as well as a photo which will appear on the badge
- Title/position
- Name of the company
- Phone number
- Note who will be picking up the badge or badges from ID Card Services
- Please also let us know if the cards will need proximity access
Basic badges cost $10 each. Access badges cost $15 each. You will receive email confirmation once the request has been processed and the badges are ready to be printed. Individuals picking up contractor or vendor badges must present a government-issued photo ID.
Any proximity access needed can be requested by the project manager or owner’s representative after the badges are picked up.
Employment Agency Badges
UO-affiliated customers use this service to request badges for employees hired through temp employment agencies, Housing custodial, Dining temps, etc. There is a $25 fee for proximity access, $15 fee for no proximity access.

In addition to producing UO ID cards, the card office also offers a number of additional services to departments.
Department Badges
ID Card Services can create photo/name badges upon request for UO departments that would like their employees to wear one. This badge is the same size and shape as the regular UO ID card and may contain your department logo or other artwork of your choice.
There is an initial set up fee of $50 and a $10 fee per badge.
Proximity Cards
Proximity cards are now a part of the University of Oregon’s physical access control system. These cards are issued by ID Card Services and programmed for access by the Key Access Office.
All university photo ID cards (UO ID card) are printed on proximity cards.
Access Cards (no photo) may be requested by departments to allow those affiliated with the department but who do not qualify for a university ID card to have building access. Access must be requested through the Key Access Office. There is a charge for these cards.
Department Copy Cards
In order to copy at the library using departmental or grant funds, a departmental copy card must be obtained at ID Card Services. There is a $1 fee for this card. To obtain a new department copy card or to make a deposit to your department’s card, you may come into the Card Office with an index number or use the service request form:
- Name of the department or specific lab that will appear on the card
- Contact person’s name, campus address, and phone extension
- Index to charge
- Amount you need added to the card
ID Card Services can mail the card to the department or hold it in the Card Office for pick up.
